It’s easy! Call one of our enrollment locations to make an appointment. They will tell you what you need to bring. A trained person called a Certified Application Assistant (CAA) will help you fill out the paper work.
Applications done with the help of a CAA are more likely to be complete. The result for you is that your child gets enrolled in a health insurance plan!

Who is Eligible
- Residents of Humboldt County.
- Uninsured children ages 0 to 19th birthday regardless of immigration status.
- A family of 4 can make up to $5,163 a month and still qualify.

What you will need to enroll
Call HealthyKids Humboldt to learn exactly what you will need to enroll. Here are examples of what you might need:
1. Proof of Humboldt County Residency:
- pay stub that includes your home address, or
- utility bills, or
- rental agreements, or
- driver’s license or state identification card
2. Proof of Income:
- copy of a recent pay stub, or
- employer statement of gross monthly income, or
- last year’s federal income tax return, or
- three month profit and loss statement (we can help you with this.)
- for those without written proof of income, the enrollment assistants can help you produce a statement of income
3. Other Information:
- birth certificates for children (only required for U.S. Citizens)
- Proof of deductions
